Patients are billed on a monthly basis.
There is a one-time registration fee of $49.
Please schedule a complimentary meeting to determine if Culver Primary is right for you.
You will never be charged any co-pays for visits nor will your insurance be billed. You will be notified ahead of time if you will be responsible for any additional costs such as procedures/immunizations/lab draws/etc.
Members receive 15% off the cash price of labs.
We ask for a 3 month initial commitment, billed monthly, when joining.
After that, you may cancel anytime with 30 days’ notice by email (info@culverprimary.com), phone (310-929-0291), or mail.
